Национальной целью развития Российской Федерации является увеличение ожидаемой продолжительности жизни при рождении до 78 лет к 2030 году [<xref ref-type="bibr" rid="cit1">1</xref>]. Достижение этой цели невозможно без снижения смертности в старших возрастных группах и увеличения продолжительности жизни пожилого населения.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Определить резервы снижения смертности пожилого населения в России и вклад пожилых возрастных групп в рост ожидаемой продолжительности жизни при рождении.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Для анализа смертности были использованы данные Федеральной службы государственной статистики (Росстата) по причинам смерти с распределением населения по полу, пятилетним возрастным группам и краткой номенклатуре причин смерти Росстата за 2022 год; данные о среднегодовой численности населения по полу и пятилетним возрастным группам за 2022 год; данные среднего варианта прогноза Росстата о численности населения по возрастным группам к 2030 году. Также использовались данные полных таблиц смертности из Human Mortality Database [<xref ref-type="bibr" rid="cit2">2</xref>] и возрастные коэффициенты смертности по причинам смерти из базы данных по смертности ВОЗ [<xref ref-type="bibr" rid="cit3">3</xref>]. Вклад изменений в смертности по возрастным группам и причинам смерти в рост ожидаемой продолжительности жизни рассчитан с помощью метода декомпозиции [<xref ref-type="bibr" rid="cit4">4</xref>]. Прогноз вклада причин смерти в увеличение продолжительности жизни в пожилом возрасте рассчитан с учетом структуры смертности по причинам смерти в 2022 году (за исключением злокачественных новообразований и класса «Симптомы, признаки и отклонения от нормы»).</p></sec><sec><title>Результаты</title><p>Результаты. С 2005 года в России наблюдался устойчивый рост ожидаемой продолжительности жизни, прерванный в 2020–2021 годах пандемией COVID-19. За период с 2005 по 2019 год ожидаемая продолжительность жизни мужчин при рождении увеличилась на 9,3 года (с 58,9 до 68,2 лет), при этом снижение смертности мужчин в возрастных группах старше 60 лет определило 21% этого роста. Ожидаемая продолжительность жизни женщин при рождении за 2005–2019 годы увеличилась на 5,7 года, вклад снижения смертности в возрасте старше 60 лет составил 48%. Большая часть снижения смертности в пожилом возрасте была определена снижением смертности от болезней системы кровообращения. При этом одновременно наблюдался рост смертности от симптомов, признаков и отклонений от нормы (класс МКБ-10, включающий причину смерти «Старость» и другие коды для неопределенных причин) и от других причин смерти, в т. ч. болезней нервной и эндокринной систем.</p><p>Вклад пожилого населения в рост ожидаемой продолжительности жизни (ОПЖ) при рождении увеличивается по мере роста этого показателя. Так, в период с 2005 по 2011 год на снижение смертности в возрасте 60 лет и старше приходилось 29% всего роста ожидаемой продолжительности жизни при рождении, в период с 2011 по 2019 год вклад снижения смертности в пожилом возрасте увеличился до 36%. По прогнозу, до 2030 года при достижении показателя ожидаемой продолжительности жизни при рождении 78 лет к 2030 году 39% всего роста показателя будет обеспечено возрастными группами старше 60 лет, в том числе почти 30% у мужчин и более 50% у женщин.</p><p>По прогнозу, до 2030 года почти половина всех резервов роста ОПЖ в возрасте 60 лет и старше приходится на класс болезней системы кровообращения, главным образом за счет ишемической болезни сердца, а также острого нарушения мозгового кровообращения и других цереброваскулярных болезней. По нашим оценкам, второй по значимости вклад в резервы роста ОПЖ вносит класс «Симптомы, признаки и отклонения от нормы» (неопределенны причины смерти, в том числе старость). Возрастные коэффициенты смертности от данного класса причин резко увеличиваются после 80 лет. Использование кодов причин смерти из этого класса занижает смертность от других причин смерти, в том числе болезней системы кровообращения, новообразований и других заболеваний [<xref ref-type="bibr" rid="cit5">5</xref>], и требуется улучшение качества кодирования и выбора первоначальной причины смерти при заполнении медицинских свидетельств о смерти. Следующие по значимости причины смерти — злокачественные новообразования и COVID-19 (около 9%). Новообразования находятся на втором месте в структуре смертности пожилого населения после БСК, вместе с тем предполагаемый вклад изменения смертности от рака в рост ожидаемой продолжительности жизни существенно ниже как болезней системы кровообращения, так и класса «Симптомов, признаков и отклонений от нормы». Ранее было показано, что в России снижение смертности от новообразований в меньшей степени является резервом по увеличению ожидаемой продолжительности жизни по сравнению с болезнями системы кровообращения и внешними причинами [<xref ref-type="bibr" rid="cit6">6</xref>]. На это также указывает и опыт других стран. Так, за период с 1970 по 2018 год снижение стандартизованного коэффициента смертности от новообразований в странах ЕС-15 составило менее 25%, тогда как от болезней системы кровообращения — более 70% [<xref ref-type="bibr" rid="cit7">7</xref>]. Оставшиеся резервы по увеличению продолжительности жизни пожилого населения распределяются между сахарным диабетом, болезнями нервной системы, болезнями органов дыхания и пищеварения и внешними причинами смерти.</p></sec><sec><title>Выводы</title><p>Выводы. По мере старения населения и роста продолжительности жизни снижение смертности пожилого населения приобретает все большее значение в контексте увеличения ожидаемой продолжительности жизни при рождении. Это определяет важность разработки целевых мер по снижению смертности в возрасте старше 60 лет. С целью увеличения продолжительности жизни приоритетными направлениями при планировании в сфере здравоохранения должны быть болезни системы кровообращения, которые определяют около половины снижения смертности в пожилом возрасте. Важное значение также имеет улучшение качества статистики по причинам смерти в этой возрастной группе, поскольку широкое использование неопределенных причин смерти не позволяет проводить детальный анализ смертности в старческом возрасте и затрудняет разработку таргетных мер по ее снижению.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. By 2030, the Russian Federation aims to increase the life expectancy at birth to 78 years as part of its national development plan [<xref ref-type="bibr" rid="cit1">1</xref>]. This target value cannot be achieved without a reduction in mortality rates among older demographics and an increase in the life expectancy of the aging population.</p></sec><sec><title>Aim</title><p>Aim. To identify reserves for reducing mortality among older population in Russia and the contribution of older age groups to the increase in life expectancy at birth.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Data from the Federal State Statistics Service (FSSS) on causes of death with population distribution by gender, five-year age groups, and brief nomenclature of death causes for 2022 were used for mortality analysis; data on the average annual population by gender and five-year age groups for 2022; and data from the average forecast variant of FSSS on population size by age groups up to 2030. Data from the Human Mortality Database [<xref ref-type="bibr" rid="cit2">2</xref>] and agespecific mortality rates by causes of death from the WHO mortality database [<xref ref-type="bibr" rid="cit3">3</xref>] were also used. The contribution of changes in mortality by age groups and causes of death to the increase in life expectancy was calculated using the decomposition method [<xref ref-type="bibr" rid="cit4">4</xref>]. The forecast contribution of causes of death to the increase in life expectancy in older population was calculated taking into account the mortality structure by causes of death in 2022 (excluding malignant neoplasms and the Symptoms, signs, and abnormal clinical and laboratory findings, not elsewhere classified class).</p></sec><sec><title>Results</title><p>Results. Since 2005, Russia has seen a steady increase in life expectancy, interrupted in 2020–2021. COVID-19 pandemic. For the period from 2005 to 2019. Life expectancy for men at birth increased by 9.3 years (from 58.9 to 68.2 years), while the decrease in mortality among men in age groups over 60 years accounted for 21% of this increase. Life expectancy of women at birth for 2005–2019. increased by 5.7 years, the contribution of the reduction in mortality over the age of 60 years was 48%.</p><p>Most of the decline in mortality in old age was determined by a reduction in mortality from diseases of the circulatory system. At the same time, there was an increase in mortality from symptoms, signs and abnormalities (ICD-10 class, including the cause of death old age and other codes for undetermined causes), and from other causes of death, incl. diseases of the nervous and endocrine system.</p><p>The contribution of the older population to life expectancy at birth increases as this indicator increases. Thus, in the period from 2005 to 2011. The decline in mortality among people aged 60 years and over accounted for 29% of the total increase in life expectancy at birth, from 2011 to 2019. Тhe contribution of reducing mortality in old age increased to 36%. According to the forecast until 2030, when life expectancy at birth reaches 78 years, by 2030 39% of the total growth in the indicator will be provided by age groups over 60 years of age, including almost 30% for men and more than 50% for women.</p><p>The forecast for 2030 shows that almost half of the potential increase in life expectancy at age 60 is linked to illnesses of the circulatory system, specifically coronary heart disease and various cerebrovascular events.</p><p>According to our estimates, the second most important contribution to life expectancy growth reserves is made by the class Symptoms, Signs and Deviations from the norm (uncertain causes of death, including old age). Age-specific mortality rates from this class of causes increase sharply after age 80 years. Using cause-of-death codes from this class underestimates mortality from other causes of death, including diseases of the circulatory system, neoplasms and other diseases [<xref ref-type="bibr" rid="cit5">5</xref>] and requires improved quality of coding and selection of the original cause of death when completing medical death certificates.</p><p>The next leading causes of death are malignant tumours and COVID-19 (about 9%). Neoplasms are in second place in the structure of mortality in older population after CSD, however, the estimated contribution of changes in cancer mortality to the increase in life expectancy is significantly lower than both diseases of the circulatory system and the class of Symptoms, signs and abnormalities. It was previously shown that in Russia, a decrease in mortality from neoplasms is to a lesser extent a reserve for increasing life expectancy compared to diseases of the circulatory system and external causes [<xref ref-type="bibr" rid="cit6">6</xref>]. Evidence from other countries between 1970 and 2018 supports a decrease of less than 25% in the standardized mortality rate from neoplasms in the EU-15 countries, while from diseases of the circulatory system it was more than 70% [<xref ref-type="bibr" rid="cit7">7</xref>].</p><p>The remaining reserves aimed at extending the life expectancy in older population are distributed among conditions such as diabetes, nervous system diseases, respiratory and digestive disorders, and external causes leading to mortality.</p></sec><sec><title>Conclusion</title><p>Conclusion: With increasing life expectancy and aging population, the focus shifts to decreasing mortality rates among older adults, in light of the rising life expectancy at birth. Thus, it is crucial to implement targeted strategies in order to decrease mortality rates among individuals aged 60 and above. In order to increase life expectancy, priority areas in health care planning should be diseases of the circulatory system, which determine about half of the reduction in mortality in older age.
